Job summary

Bristol Hospice is seeking a Clinical Supervisor to ensure high-quality hospice care delivery and supervise clinical personnel. This role will coordinate case referrals, assign staff, and collaborate with the Medical Director to assess eligibility for services.

The ideal candidate has an RN license in good standing, 2+ years in hospice supervision, CPR certification, and strong communication skills. This position offers opportunities for professional development and involvement in QAPI

Qualifications

• RN license in good standing with the state where practicing.
• Two years of management or supervisory experience in hospice/home care.
• Current CPR certification is required.
• Valid driver’s license and reliable transportation.
• Strong verbal and written communication abilities.

Responsibilities

• Receive case referrals and review patient information for hospice/home care needs.
• Assign hospice personnel to referrals as needed.
• Confer with Medical Director and Attending Physician on eligibility for services.
• Review cases through home visits, records review and clinician reports.
• Guide clinicians to improve quality and delivery of home care services.
• Be available during hours to assist clinicians as appropriate.
• Conduct clinical record reviews and report findings to the Clinical Director.
• Assist in screening and interviewing new employees and provide recommendations.
• Assist in orientation of new staff and ongoing education programs.
• Support policy development and organizational planning efforts.
• Ensure compliance with professional standards and QAPI program.
• Promote customer service orientation and active community involvement.
• Participate in quality improvement teams as directed.
