Job summary

The Roads Home Health Care is seeking a Home Health Medical Social Worker (MSW) to provide psychosocial assessment and support to patients and families across South Florida. Both full-time and per diem opportunities are available.

You will perform in-home psychosocial assessments, coordinate care with physicians and families, educate on resources and coping strategies, and deliver interventions tailored to each patient's needs.

Qualifications

• Must hold a current, valid Florida clinical social worker license.
• Minimum one year of post-master's experience in a clinical setting.
• Ability to pass a criminal background check and maintain CPR/BCLS certification.

Responsibilities

• Perform comprehensive psychosocial assessments in patients' homes
• Obtain and communicate physician orders for certification and recertification of medical social work services
• Coordinate care planning with physicians, patients, families, and the interdisciplinary care team
• Educate patients and families on available resources and coping strategies
• Deliver social work interventions appropriate to each patient's needs and clinical status
• Identify safety, environmental, and mobility risks in the home and recommend interventions
• Connect patients and families with community resources to support optimal outcomes
• Support transfer and discharge planning in accordance with agency policy
• Complete timely, accurate documentation and participate in interdisciplinary team conferences
• Supervise social work assistants and support the team in delivering coordinated patient care

Job Requirements

• Current, valid Florida clinical social worker license
• Master's or doctoral degree from an accredited school of social work
• Minimum one year of post-master's experience in a clinical setting
• Current CPR/BCLS certification, current health certificate, and TB clearance
• Valid driver's license, reliable vehicle, and current auto insurance; ability to pass a criminal background check

• Excellent interpersonal and communication skills for building rapport with patients and families
• Ability to work independently while managing a home visit schedule
• Compassionate, culturally sensitive approach to patient and family support
• Strong organizational and documentation skills
• Genuine commitment to advocating for patients and connecting them with the support they need
• Prior home health experience preferred
• Bilingual candidates a plus
