Job summary

Bristol Hospice - Sacramento Llc is seeking a Clinical Supervisor to lead clinical staff, assign referrals, and oversee patient care in our California hospice program.

You will collaborate with Medical Directors, conduct case reviews, and participate in education and policy initiatives to ensure high-quality home care services.

Qualifications

• Registered Nurse license in good standing to practice nursing in the state
• Hospice home care experience with two years in management or supervisory role
• Excellent observation, verbal, and written communication skills
• Valid driver’s license
• Current CPR certification

Responsibilities

• Receive case referrals and review patient information to determine hospice/home care needs
• Assign appropriate hospice personnel to referral cases
• Confer with Medical Director and Attending Physician regarding eligibility for services
• Review cases through home visits, records review, and services provided by clinicians
• Conduct clinical record reviews and communicate findings to Clinical Director and staff
• Assist in screening and interviewing new employees and orientation of new staff
• Assist in planning, implementing and evaluating education programs
• Ensure compliance with organizational policies and QAPI program and improve care quality
