Responsibilities

• Provide pharmaceutical care including medication order review, preparation, dispensing, documentation and monitoring.
• Counsel patients and collaborate with physicians, nurses, pharmacists and other health professionals to ensure safe and effective patient care.
• Intervene on orders as warranted and participate in medication reconciliation and disease management in assigned clinical areas.
