Qualifications

• Bachelor's degree required.
• Doctor of Pharmacy (PharmD) preferred.
• Progressive pharmaceutical care experience or Pharmacy Practice Residency preferred.

Responsibilities

• Review, prepare, dispense medications and provide patient counseling.
• Collaborate with physicians, nurses, pharmacists and other health professionals to deliver safe, effective patient‑centered care.
• Interpret, review and process medication orders, intervene as warranted; monitor and reconcile medications.
