Responsibilities

• Monitor pharmacotherapy plans and adjust based on labs, patient status, and clinical guidelines.
• Participate in interdisciplinary rounds and collaborate with medical, nursing, and ancillary teams.
• Perform medication reconciliation and support transitions-of-care workflows.
• Respond to medication-related inquiries from providers and clinical staff.
• Document clinical interventions and maintain accurate pharmacy records.
• Ensure compliance with internal policies, formulary guidelines, and quality standards.
• Support antimicrobial stewardship and other clinical initiatives as assigned.

A well-established community hospital in northern California is seeking a Clinical Staff Pharmacist for a 13-week acute-care assignment. Work Environment & Schedule
