Responsibilities

  • Provide comprehensive clinical and operational pharmacy services within an acute-care hospital.
  • Verify medication orders for accuracy, safety, and therapeutic appropriateness.
  • Prepare, compound, and dispense medications in accordance with regulatory standards.
  • Monitor pharmacotherapy plans and adjust based on labs, patient status, and clinical guidelines.
  • Participate in interdisciplinary rounds and collaborate with medical, nursing, and ancillary teams.
  • Perform medication reconciliation and support transitions-of-care workflows.
  • Respond to medication-related inquiries from providers and clinical staff.
  • Document clinical interventions and maintain accurate pharmacy records.
  • Ensure compliance with internal policies, formulary guidelines, and quality standards.
  • Support antimicrobial stewardship and other clinical initiatives as assigned.

Clinical Staff Pharmacist – Acute Care Hospital (13-Week Contract)
Overview

A well-established community hospital in northern California is seeking a Clinical Staff Pharmacist for a 13-week acute-care assignment. This role supports medication verification, sterile and non-sterile preparation, clinical reviews, and collaboration with the inpatient care team. Work Environment & Schedule
