Job summary

Qualifications

• PharmD or BS Pharmacy required.
• Acute-care hospital pharmacy experience strongly preferred.
• Knowledge of sterile compounding, EMR systems, and hospital medication-use processes.
• Strong communication, clinical judgment, and problem-solving abilities.

Responsibilities

• Provide comprehensive clinical and operational pharmacy services within an acute-care hospital.
• Verify medication orders for accuracy, safety, and therapeutic appropriateness.
• Prepare, compound, and dispense medications in accordance with regulatory standards.
• Monitor pharmacotherapy plans and adjust based on labs, patient status, and clinical guidelines.
• Participate in interdisciplinary rounds and collaborate with medical, nursing, and ancillary teams.
• Perform medication reconciliation and support transitions-of-care workflows.
• Respond to medication-related inquiries from providers and clinical staff.
• Document clinical interventions and maintain accurate pharmacy records.
• Ensure compliance with internal policies, formulary guidelines, and quality standards.
• Support antimicrobial stewardship and other clinical initiatives as assigned.
