Job summary

Sweetwater Care in California is seeking a Speech-Language Pathologist to assess and treat patients with speech, language, cognitive and swallowing disorders in a skilled nursing facility. You will evaluate and treat patients, plan goals, educate families, and document progress in accordance with guidelines.

You will develop and implement treatment plans, monitor progress, and collaborate with physicians, therapists and staff to ensure high-quality care and compliance with reimbursement

Qualifications

Responsibilities

• Direct all aspects of quality patient care, including evaluation and treatment, treatment planning, goal setting, family education and documentation according to company guidelines.
• Provides professional treatment while using therapeutic techniques for the rehabilitation of patients with speech, language, hearing, oral motor, swallowing and cognitive disorders.
• Plans activities and provides therapeutic services for speech and language disorders.
• Serves as a patient and rehabilitation advocate; participates in quality assurance and customer service activities.
• Perform patient evaluations and develop treatment plans, including short-term and long-term goals, consistent with physician orders.
• Collaborate with other therapists and physicians relative to patient treatment programs and progress.
• Document patient evaluations, progress notes, and discharge summaries for medical records in a timely manner.
• Provide a full range of speech therapy modalities and assist with clinical affiliations of speech therapy assistants.
