Essential Functions

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.

Qualifications

• Master's Degree Social Work or Master's Degree Mental Health & Behavioral Medicine required
• Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LICSW) or Licensed Mental Health Counselor (LMHC) required
• 0-1+ years of clinical experience in medical setting required.
• Pediatric experience strongly preferred.
• Bilingual (Spanish/English) strongly preferred.

• Aware of opportunities in department/service initiatives; participates in initiatives at an entity level. Uses data to measure progress.
• Demonstrate fundamental skills under supervision. May need assistance in promoting collaboration among healthcare team members, other colleagues, and the organization to support and enhance patient care.

Schedule and Work Model: Monday through Friday ET, 8am-4:30pm or 8:30am-5pm. Must be flexible to support on-site needs at medical practices in Chelsea, Revere and Charlestown (approximately 2-3x/week but may vary based on case load) and at patient homes or schools for meetings. 2x monthly team meetings at Assembly Row in Somerville, MA office required (1st and 3rd Wednesdays of each month). Remote work requires stable, secure, quiet, compliant work station. Pay Range $55,390.40 - $80,485.60/Annual Grade 6
