Job summary

Responsibilities

• Perform psychosocial assessments and develop/implement care plans.
• Educate and consult with the healthcare team on psychosocial aspects of care.
• Advocate, mediate and coordinate with families and patients across the continuum of care.
• Facilitate transitions/discharge planning to appropriate care settings.
• Provide education and support to patients, families and staff on psychosocial issues.
