Qualifications

Responsibilities

• Provides social work and behavioral health services for medical home clinic patients collaboratively with care team.
• Contributes to patient and family/caregiver engagement in care planning.
• Works in accordance with policy and procedures of Providence Medical Group Montana and St. Patrick Hospital and Health Sciences Center.
