Job summary

HarmonyCares is seeking a Clinical Social Worker to join our home-based care team. You will manage a caseload of high-risk socially complex patients, conduct face-to-face and telephonic psychosocial assessments, and design interventions to close gaps in care while coordinating services with the care team.

Qualifications

• Master’s degree in social work is required.
• Must be licensed as an LCSW or LISW depending on state.
• Must have a valid driver’s license and good driving record.
• Ability to use sound clinical judgment and communicate clearly in both written and verbal formats.
• Above average computer skills.
• Self-directed and able to communicate effectively across disciplines.
• Ability to plan time, balance multiple tasks, and implement changes.

Responsibilities

• Manage a caseload of high-risk socially complex patients.
• Conduct face-to-face and telephonic psychosocial assessments with patients and caregivers.
• Investigate barriers and design interventions to close care gaps.
• Provide short-term counseling as needed.
• Coordinate services to ensure patients understand their treatment plan.
• Collaborate with the care team to achieve quality and patient experience outcomes.
