Qualifications

Responsibilities

• Interviews patients and relatives to obtain social history relevant to medical problems and planning.
• Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care.
• Liaises between medical and nursing staff, patients, relatives and outside agencies.
• Interpret and resolve social problems related to medical condition and hospitalization.
• Social Worker II assists with discharge coordination to ensure a successful transition in collaboration with the health care team.
• Conducts assessments during hospitalization and outpatient follow-up; identifies risk factors after discharge.
• Bridges discharge to outpatient provider addressing issues with patient and caregiver.
