Job Summary

This position is responsible for collecting and evaluating information about patients; establishing treatment goals with clients; determining treatment methodologies; suggesting and exploring resolutions; discussing progress toward goals; evaluating and adjusting treatment provided; supervising student practice and caseload management.

Essential Functions
  • Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
  • Refer patient, client, or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education.
  • Plan discharge from care facility to home or other care facility.
  • Plan and conduct programs to combat social problems, prevent substance abuse, or improve community health and counseling services.
  • Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.

Qualifications
Education

Bachelor's Degree in Social Work required or Master’s Degree in Social Work required or Master’s Degree in Mental Health & Behavioral Medicine required

  • Class D Passenger Vehicle Driver's License [State License] preferred
  • Social Worker, Licensed Clinical [LCSW - State License] preferred

  • Licensed Mental Health Counselor [State License] preferred
Experience

Clinical experience in medical setting 0-1 year required
