Job summary

PeaceHealth is seeking a Clinical Social Worker for a Part Time, 0.80 FTE, Day position in Oregon. The role focuses on developing and coordinating patient care plans through clinical assessment and multidisciplinary collaboration.

Responsibilities include intake, treatment planning, discharge planning, risk assessment, case management and coordination of services to ensure continuity of care. Credentials LCSW/CSWA/LMSW (OR) are required.

Qualifications

• Master’s Degree in Social Work from an accredited graduate school
• 2 years of clinical experience with designated population ( Required )
• Credentials: LCSW (OR), CSWA (OR), or LMSW (OR)
• Experience in hospital, ER, chemical dependency, or community agency settings preferred
• Knowledge of Medicare/Medicaid and insurance requirements related to continuing care (Preferred)
• Ability to supervise graduate social work students toward licensure and provide training

Responsibilities

• Coordinate development and implementation of patient care/treatment plans.
• Conduct diagnosis and clinical assessments for psychiatric, chemical dependency, and psychosocial needs.
• Provide discharge planning, risk assessment, case management, and care continuity coordination.
• Perform intake documentation and ongoing assessments; collaborate with multidisciplinary treatment team.
• Provide brief therapeutic interventions and participate in team staffings.
• Ensure privacy and confidentiality compliance; interact with legal entities as needed.
• Contribute to department quality improvement efforts.
• Provide instruction and supervision for graduate social work students toward licensure.
