Lakeshore Meadows is seeking a dedicated Social Worker to join our team in Nashville, TN. The role requires a Bachelor’s degree and emphasizes case management within SNF/ICF settings, with knowledge of MDS and clinical assessments.

The successful candidate will coordinate care with an interdisciplinary team, assist with discharge planning and transition back to the community, and advocate for patients while maintaining confidentiality.

Qualifications

• Bachelor's degree from an accredited program or related field is required.
• Experience preferred with case management in SNF/ICF setting and knowledge of MDS and clinical assessments and documentation.
• Experience with care planning, discharge planning, and collaboration with hospice, home health and mental health services.

Responsabilités

• Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
• Working with patients and representatives toward transition back to the community or adjustment to center life and navigating illness/disability crises
• Advocating for patients and representatives to express and resolve grievances
• Coordinating patient care plans with interdisciplinary team members
• Reporting to the Director of Social Work on case management status and needs
• Maintaining confidentiality of professional information
