Job summary

Elica Health Centers is seeking a Behavioral Health Specialist to join our team in California. You will provide brief interventions, warm handoffs, and crisis support within the PCBH model while collaborating with PCPs and BH colleagues to coordinate care.

Responsibilities include timely documentation, outreach for follow-up, evidence-based assessments, and ensuring privacy and ethical standards in a diverse community setting. A MSW with active ASW registration is required.

Qualifications

• Active ASW registration in good standing with the California Board of Behavioral Sciences.
• Master of Social Work (MSW) from an accredited program.
• Experience providing brief, evidence-based interventions in clinical or community settings.
• Strong oral, written, and electronic communication skills.
• Demonstrated ability to work effectively with diverse populations and multidisciplinary teams.
• Knowledge of HIPAA, privacy laws, and the NASW Code of Ethics.

Responsibilities

• Provide same-day warm handoffs and brief, evidence-based interventions.
• Collaborate with PCPs, MAs, and BH team; refer to internal/community services.
• Provide de-escalation, safety planning, and crisis intervention.
• Document patient encounters and case consultations in a timely manner.
• Manage BH inbox/voicemail and scheduling; conduct outreach for follow-up care.
• Conduct behavioral health screenings and follow-ups; participate in BH team meetings and quality improvement.
• Ensure adherence to HIPAA, privacy, consent, and ethical standards.
• Support patient engagement and culturally responsive care.
