NHC HealthCare Franklin is seeking a Clinical Social Worker Admissions Coordinator / Discharge Planner for full-time employment at our Franklin, TN location. The role requires a bachelor’s or master’s degree in social work or related humanities, with preferred experience in case management within SNF/ICF or acute care settings.

Qualifications

• Bachelor’s or Master’s degree in social work or related humanities (sociology/psychology)
• Experience preferred with case management in SNF/ICF or acute care; knowledge of MDS, clinical assessments, documentation and care planning
• 1:1 case management with patients and their support systems

Responsibilities

• Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
• Working with individual patients and their representatives toward transition back to the community or adjustment to center life
• Helping navigate a patient’s crisis of illness and/or disability
• Serving in an advocacy role for patients and their representatives in expressing and resolving grievances
• Coordinating patient’s plan of care with interdisciplinary team members
• Reporting to the Director of Social Work current case management status and needs
• Maintaining confidentiality of professional information
