Job summary

NHC Healthcare Franklin is interviewing for a Clinical Social Worker (Full Time). The qualified applicant should hold a bachelor’s or master’s degree in social work or related humanities, with SNF/ICF or acute care experience and knowledge of MDS, assessments, care planning and discharge planning.

Responsibilities include strong clinical and customer service within a fast-paced setting, coordinating care with the inter-disciplinary team, and advocating for patients and their families while

Qualifications

• Bachelor’s or Master’s degree in social work or related humanities.
• SNF/ICF or acute care case management experience preferred.
• Knowledge of MDS, clinical assessments, care planning and discharge planning.
• Experience with support services (hospice, home health, mental health) is a plus.

• Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment.
• Working with patients and representatives toward transition back to the community or center life.
• Helping navigate a patient’s crisis of illness and/or disability.
• Advocating for patients and their representatives in expressing and resolving grievances.
• Coordinating patient’s plan of care with interdisciplinary team members.
• Reporting current case management status and needs to the Director of Social Work.
• Maintaining confidentiality of professional information.

Position: Clinical Social Worker - Full Time

NHC Healthcare Franklin is interviewing for a clinical social worker and the qualified applicant should hold a bachelor’s or master’s degree in social work or a degree in sociology or psychology or related humanities. Experience preferred with a case management background in a SNF / ICF or acute care setting, has knowledge of MDS, clinical assessments and documentation, care planning, discharge planning, working with support agencies such as hospice, home health and mental health services, and 1:1 case management with patients and their support systems.

Pay Rate: 50k-60k plus incnetives

Responsibilities Include, But Are Not Limited, To

• Exhibiting strong clinical and customer service skills in a fast-paced healthcare environment
• Working with individual patients and their representatives toward transition back to the community or adjustment to center life
• Helping navigate a patient’s crisis of illness and / or disability
• Serving in an advocacy role for patients and their representatives in expressing and resolving grievances
• Coordinating patient’s plan of care with interdisciplinary team members
• Reporting to the Director of Social Work current case management status and needs
• Maintaining confidentiality of professional information
