Job summary

Job description

Under the supervision of the Clinical Supervisor for the Citywide Supportive Housing Program, the Clinical Social Work I/II (CSW I/II) works with San Franciscans living in Permanent Supportive Housing who struggle with issues of mental and physical health, substance use, and housing retention. The role collaborates with community providers and links residents to appropriate outpatient services and provides housing stabilization to prevent homelessness.

Responsibilities
  • Carry a caseload of 25 residents from one of the DISH supportive housing sites (Camelot, Le Nain, Empress, Windsor, Star, Pacific Bay Inn).
  • Provide individual treatment including case management, psychotherapy, crisis intervention, 5150 assessments, substance use treatment, psycho‑education, group treatment, and community events.
  • Conduct intake, assessment, service planning, and consultation.
  • Maintain client records in accordance with California, City, and County requirements and coordinate treatment strategies with property management, DPH Nursing, and other service providers.
  • Track client contacts in Epic and One Systems and provide referrals and linkage to hospitals and clinic medical providers.
  • Attend weekly staff meetings and participate in required training sessions.
  • Demonstrate commitment to anti‑racism, social justice, and the ability to engage and respect a diverse community.
  • Perform any additional duties as assigned.
Minimum Requirements
  • Master’s degree in social work, psychology, or a related field from an accredited university.
  • For CSW I: Registered as an Associate Clinical Social Worker (ASW) or Intern Marriage‑Family Therapist (IMFT) with the Board of Behavioral Science.
  • For CSW II: Possession of the requirements necessary for application for licensure as determined by the California Board of Behavioral Science.
  • Ability to work with clients and program staff from diverse cultural and social class backgrounds.
  • Understanding of mental health issues and sensitivity toward mental health clients.
  • Ability to work cooperatively, communicate effectively, and work well under pressure and with deadlines.
  • Strong written and verbal communication skills, including the ability to prepare written summaries and reports within required time frames.
  • Knowledge of medical and/or psychiatric terminology.
  • Excellent attendance and reliability.
  • Demonstrated ability to lift 50 lbs and climb stairs.
  • Preliminary job offer contingent upon successful background clearance.
  • Two years of experience working with severely mentally ill adults, the homeless, and dual‑diagnosed populations.
  • One year of experience in community‑based case management, including outreach to SRO hotels and scattered site housing locations.
  • Case management experience.
Preferred Attributes
  • Problem‑solving skills.
  • Intensive outreach and engagement to link and treat clients with long histories of mental illness and homelessness who are reluctant to engage in treatment.
  • Engagement of formerly homeless individuals in services to improve housing stability.
  • Ability to find safe and affordable housing for clients with behavioral and substance‑use problems.
  • Ability to report to Adult Protective Services when there is evidence of abuse.
  • Ability to contact supervisor if the employee or client is harmed or if there is a concern regarding client care.
Required Licenses and Certifications
  • BBS Licensure (e.g., LCSW, LPCC, MFT).
  • BBS registration.
