Job Summary
Under the direction from the supervising social worker, the incumbent will work with individuals living with HIV (or affected by HIV) and families to provide low threshold/low barrier social work services within an ambulatory care setting. These services include, but are not limited to: psychosocial assessments, assisting patients in referrals to emergency and long-term housing stabilization, substance use counseling, mental health assessments, and appropriate referrals to community agencies for financial benefits, legal, and immigration resources. The incumbent will provide a full array of social services that include, but are not limited to: crisis interventions, provider consultations, insurance, and financial counseling, and participate in multi-disciplinary case conferences on an ongoing basis; in which care plans are developed for high-risk patients. The incumbent will be required to participate in social work meetings, staff meetings, case conferences, and other required Ward 86 Clinic meetings. The incumbent may be responsible for providing culturally specific care to monolingual Spanish-speaking patients. The incumbent may also have a special responsibility in serving patients over 50 years of age at Ward 86 Clinic in our HIV and Aging program.

The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $37.56 - $50.31 (Hourly Rate).

ABOUT WARD 86 CLINIC
Ward 86, the HIV/AIDS Clinic at the Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG) campus has been a leader and innovator in field of HIV care since the beginning of the epidemic. Ward 86 is the largest clinic providing primary medical care for people living with HIV for the San Francisco Health Network (SFHN) and also provide preventive services to patients at-risk for HIV infection, including pre-exposure prophylaxis (PrEP). Ward 86 provides care to approximately 2400 patients living with HIV, with over 22,000 clinical encounters a year. Services are offered through a comprehensive, multi-disciplinary approach, addressing primary medical and HIV care, urgent care, mental health evaluation and treatment, substance use assessment, treatment and referrals, HIV prevention services, and HIV specialty care needs. Ward 86 has expanded its footprint by providing care to the city’s most vulnerable population of patients living with HIV and those at-risk of HIV using a drop-in model on Ward 81 South. Ward 81 South is now the location of the POPUP Clinic and the Heath Access Point (HAP) Clinic along with the Bridge Clinic at Family Health Center.
The Division of HIV, Infectious Diseases and Global Medicine at UCSF is an internationally recognized global leader in clinical care, research and education. With its home-base located at ZSFG, the Division has been ranked as the top facility in the country for AIDS care for over ten years. The Division’s work is featured frequently in the media, and it is often visited by international, national, state and local dignitaries and delegations. The Division consists of approximately 36 faculty members and 140 academic and staff employees. It is the largest Division of the Department of Medicine at ZSFG, with a total expenditure budget of approximately $45 million dollars.
