Glengariff Rehabilitation and Healthcare Center in New York seeks a Speech Language Pathologist (SLP) to assess patients requiring rehabilitation and provide direct care. You will develop treatment plans, set goals, document progress, and educate patients and families within an interdisciplinary team.

Responsibilities include conducting evaluations, maintaining weekly notes, ensuring discharge documentation, and collaborating to optimize facility-wide programs and patient outcomes.

Formación

• Graduate from an accredited Speech-Language Pathology program.
• Valid NY license/registration required.
• Experience with long-term or geriatric care preferred.
• Strong documentation/writing skills.
• Proficient in evaluations, progress notes, re-certifications, and discharge documents.
• Collaborates with all disciplines to provide patient-centered care.

Responsabilidades

• Assess referred patients for rehabilitation needs and provide direct patient care.
• Establish and administer treatment plans with goals aligned to patient tolerance.
• Document patient conditions and educate patients and families.
• Collaborate with an interdisciplinary team to improve facility programs and processes.
