Job summary

Glengariff Rehabilitation and Healthcare Center is seeking a Speech Language Pathologist (SLP) in New York. You will assess patients requiring rehabilitation, establish and administer targeted treatment programs, and document patient progress while educating families.

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must have a valid professional license and registration for state of NY.
• Experience with long-term care and/or geriatric care is preferred but not limited to.
• Well versed with documentation writing.
• Evaluations, weekly progress notes, re-certification and discharge documentations.
• Collaborates with all disciplines to provide best patient centered treatment and care.
• Exceptional patient service skills and ability to demonstrate clinical excellence.
• Report to work on time; adhere to scheduled hours and demonstrate professionalism at all times.
• Perform all other duties assigned by Director of Rehabilitation
• Compliance with confidentiality, HIPPA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred however not limited to.

Responsibilities

• The Speech Language Pathologist assesses referred patients requiring rehabilitation services and provides direct patient care.
• Establishes and administers a treatment program with goals based on patient capacity and tolerance.
• Documents patient condition and educates patients and families in therapy methods.
• Collaborates with interdisciplinary team to improve life and function of all patients.
