Job summary

Glengariff Rehabilitation and Healthcare Center in New York seeks a Speech Language Pathologist (SLP) to assess referred patients, develop and administer treatment plans, and document patient progress under physician direction. The SLP will work as part of an interdisciplinary team to improve patients’ functional abilities in a sub-acute and long-term care setting.

The ideal candidate will be licensed in NY, have an accredited degree in Speech-Language Pathology, and experience in geriatric care

Qualifications

• Must have graduated from an accredited Speech Therapist college or university.
• Must hold a valid NY state license and registration.
• Experience with long-term care and/or geriatric care is preferred.
• Strong documentation writing skills.
• Responsible for evaluations, weekly progress notes, re-certification and discharge documentation.
• Collaborates with all disciplines for patient-centered care.
• Demonstrates exceptional patient service and clinical excellence.
• Punctuality and professionalism in scheduling.
• Performs duties assigned by Director of Rehabilitation.
• Complies with confidentiality, HIPAA and Federal Residents Rights.
• Experience with Case Mix Index and PPS scheduling is preferred.
