JOB SUMMARY:

This role provides comprehensive case management support to older adults (55+) struggling to maintain independent living in their community. Supporting clients through DAS and Enhanced Care Management (ECM) programs, this position develops and implements individual service plans, coordinates community referrals, advocates for clients, and maintains up-to-date service documentation.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
  • Conduct screenings, clinical assessments, home visits, and provide comprehensive case management services, including collaborative treatment planning and service linkage.
  • Ensure timely and accurate documentation of client interactions, progress, and outcomes in accordance with established protocols.
  • Ongoing coordination of support services and resource development
  • Conduct community outreach upon receiving client referrals
  • Maintain electronic medical records.
  • Follow all agency protocols, participate in all required agency meetings/trainings, and clinical supervision.
  • Manage and maintain an assigned caseload.
    • Escort clients to and from appointments and coordinate transportation.
    • Make referrals to appropriate social services agencies.
    • Monitor client(s) benefits for timely renewals.
    • In home/in person visits with clients.
  • Perform other duties as assigned.
JOB REQUIREMENTS:
  • Bachelor's degree in social or a Bachelor's degree in a related field with 2 years of experience.
  • 2-3 years of experience working with older adults preferred.
  • Able to work with a team of diverse and multi-disciplinary professionals.
  • Proof of negative or nonactive TB required. Curry Senior Center will provide or reimburse for the cost of a TB test after an offer of employment has been accepted by both parties. Proof of a negative TB test within 90 days will also be accepted.
KNOWLEDGE, SKILL, AND ABILITIES:
  • Must be a self-starter with excellent follow-through skills.
  • Willingness to work in a community-based setting in the Tenderloin neighborhood with low-income, frail and culturally diverse older adults who may have multiple behavioral and physical health problems.
  • Ability to work with a team of diverse and multi-disciplinary professionals.
  • Ability to maintain a professional demeanor with good interpersonal and communication skills
  • Effective oral and written communications skills with the ability to express ideas in a clear, thorough, concise manner.
  • Strong interpersonal, communication and organizational skills. Solid aptitude for customer service.
  • High degree of skill in grammar, spelling and punctuation.
  • Ability to maintain the confidentiality of all information obtained.
  • Strong organizational skills and the ability to prioritize workflow.
  • High level of analytical problem-solving skills.
  • Excellent time management skills, including the ability to manage self-development.
  • Must be action-oriented and have the ability to work independently and as a team player.
  • Strong computer skills such as Chrome, Google suite, Apple iOS, Office suite.
PHYSICAL REQUIREMENTS/DEMANDS:

The incumbent is in a non-confined office-type setting in which they are free to move about at will. The Environment is mostly clean and comfortable but may include some minor annoyances such as noise, odors, drafts, etc. Work will be performed utilizing a computer, sitting at a desk/chair or standing while speaking and interacting with clients. Position will require average finger dexterity to operate normal office equipment and systems.
