Qualifications

• Current Utah RN license.
• Home Health leadership experience.
• Knowledge of CMS quality measures and 5-Star ratings.
• Strong coaching and staff development skills.

Responsibilities

• Oversee Home Health and Hospice clinical operations and partner with Administrator to drive clinical quality and patient outcomes.
• Ensure OASIS accuracy and robust clinical documentation.
• Lead CMS quality measures and CMS compliance to support 5-Star performance.
• Develop the clinical staff and drive accountability and engagement.
• Foster interdisciplinary collaboration and strong clinical processes.
