Job summary

Your Health is seeking a Clinical Scheduler to support our Okatie Clinic Care Manager role. This full-time position coordinates all in-person visits for an interdisciplinary care team to ensure timely, appropriate encounters under our proactive care model.

The role is based daily in the Okatie Clinic office, working 8 hours, 8:00 AM–5:00 PM, Monday through Friday. Join a physician-led group serving SC and GA with strong benefits and opportunities for career development.

Qualifications

• High school diploma or equivalent required; healthcare degree preferred.
• CMA, LPN or RN certifications preferred.
• Experience in medical setting, scheduling or administrative role.
• Familiar with medical terminology and insurance verification.
• Strong organizational, time management and multitasking abilities.
• Excellent communication and interpersonal skills; able to build relationships at all levels.
• Strong problem-solving abilities.
• Ability to work effectively under pressure.

Responsibilities

• Register, verify and timely schedule patients according to the Company’s care model.
• Ensure new patients are tracked and followed until scheduled.
• Schedule appointments based on care model including follow-ups with providers.
• Ensure all disciplines with schedules are fully optimized.
• Follow patients after discharge from inpatient facilities and schedule accordingly.
• Schedule TCM appointments promptly within the required window.
• Maintain strong documentation.
• Utilize multiple software systems to track follow-up and improve outcomes.
• Interact with patients via email, portal, telephone, text, in-person to gather information and provide pre-appointment instructions.
