Responsibilities

• Assist healthcare professionals in delivering patient care.
• Monitor, record, and communicate patient condition to care team, physician, patient, and family.
• Coordinate patient care across disciplines, including transfers, discharge, referrals, and spiritual/psychosocial support.
• Evaluate learning needs of patient and/or family and provide education appropriate to age, culture, condition, and circumstances.
• Advocate for the physical and emotional well-being of the patient.

What You'll Do:

• Assist healthcare professionals in delivering patient care
