Qualifications

• Discharge planning responsibilities including coordination with home care, DME, hospice, and community resources.
• Assess patient/family needs and coordinate with the health care team.
• Educate patients about scope and limits of health care and insurance benefits.
• Maintain patient privacy and confidentiality in all communications.
• Document discharge plans and assessments in the patient record (EPIC).

Responsibilities

• Discharge planning activities: timely coordination of home care, DME, placements, hospice, community resources, transportation, and other discharge planning needs as they arise.
• Work with patient and family to assure provision of choice in post hospital setting.
• Educate patient / customers regarding scope and limitations of health care / insurance benefits.
• Respect each patient’s / customer’s personal dignity, right to privacy and confidentiality.
• Communicate expected discharge time frame.
