Job summary

PowerBack Rehabilitation in New Hampshire seeks an experienced MDS Coordinator to lead clinical assessments, care planning and MDS transmission in accordance with federal/state rules. You will collaborate with the interdisciplinary team and Business Office to optimize case-mix reimbursement and ensure compliance.

The ideal candidate is an LPN with 1+ year in long-term care, current CPR/BLS, and strong communication skills to educate staff on MDS processes and terminology.

Qualifications

• Graduate of an accredited School of Nursing with current LPN licensure in NH.
• At least 1 year of long-term care nursing experience required.
• Experience with Medicare/Medicaid reimbursement or MDS is a plus.
• CPR/BLS certification current.

Responsibilities

• Coordinate and complete clinical assessments and MDS, per regulations.
• Manage case-mix documents to ensure appropriate reimbursement.
• Conduct concurrent MDS reviews to maximize RUG categories.
• Communicate case-mix data needs to the Business Office.

Responsibilities

• The MDS Coordinator is responsible for the coordination and completion of clinical assessments, supporting documentation, care planning and transmitting the MDS, as required by federal and state regulations.
• Manage all Medicare/Medicaid case-mix documents at the nursing center to assure appropriate reimbursement for services provided. This will involve collaboration with the center interdisciplinary team.
• Conduct concurrent MDS reviews to assure achievement of maximum allowable RUG categories.
• Communicate with the Business Office Manager regarding case-mix data required for billing.
• Educate the interdisciplinary team on case-mix and the nursing staff in terminology, language, and format that is required by MDS

Qualifications

• Must be a graduate of an accredited School of Nursing with current LPN licensure in the state of employment.
• One year of long-term care clinical nursing experience is required.
• Experience with Medicare/Medicaid reimbursement, MDS completion, clinical resource utilization and/or case management is highly desirable.
• Must maintain current BLS/CPR certification.
