Job Summary

The Clinical Registered Nurse supports the care enhancement program by providing high-quality telephonic support efficiently and cost-effectively, ensuring excellent care. The nurse collaborates with Primary Care Providers and other healthcare team members to manage healthcare effectively, ensuring resource utilization and quality outcomes. Responsibilities include early identification of at-risk participants and intervening per disease management guidelines.

Supervised by an Operations Manager, all clinicians participate in orientation, competency reviews, and annual testing.

Available Shifts
  • Monday-Friday: 7am-3:30pm CT and every 3rd Saturday from 8am-4:30pm CT.
  • Monday-Friday: 11am-7:30pm CT and every 3rd Saturday from 8am-4:30pm CT.
  • Monday-Friday: 11:30am-8pm CT and every 3rd Saturday from 8am-4:30pm CT.
Training

Four weeks of training from 8am–4:30pm CT. Attendance is mandatory during the first 60 days. Hours are in Central Standard Time.

Qualifications
  • Compact nursing license required.
  • High School diploma and Nursing Diploma, Associate’s or Bachelor’s Degree in Nursing (BSN preferred).
  • Minimum 3 years recent clinical experience.
  • Knowledge of physiology and chronic diseases.
  • Effective communication skills for telephonic education and engagement.
  • Basic computer skills and familiarity with Windows OS.
Essential Job Functions
  • Review participant health exams and initiate interventions.
  • Contact participants for assessment, education, and goal setting.
  • Identify educational needs and facilitate learning materials.
  • Assess functional status and support systems.
  • Develop and implement comprehensive care plans.
  • Address barriers to healthcare access and coordinate with providers.
  • Ensure continuity of care through team collaboration.
  • Handle warm transfer calls for further assessment when needed.

Additional Requirements
  • Current RN license valid in the state of employment, with ability to obtain licenses elsewhere as needed.
  • Minimum 3 years recent clinical experience.
  • Knowledge of physiology and chronic disease management.
  • Strong communication, math, reasoning, and computer skills.
  • Ability to educate and promote behavior change telephonically.
