Position Summary
Responsible for handling tasks related to the review and timely processing of incoming referrals. Assists the Clinical Referral Coordinator III in regards to mutual patient care, with focus on co-managed and expedited patients. Position works remotely and/or in-office and provides clinical support for the referral team as related to appropriate scheduling timeframes and any necessary preparation in advance of the clinic appointment for all subspecialty patients referred into EANM. Has achieved a minimum of the Certified Ophthalmic Assistant (COA) certification with minimum 7 years of clinical experience OR (COT)/Certified Ophthalmic Medical Technician (COMT) level with minimum 3-5 years clinical experience
Essential Functions/ Responsibilities
Refer to essential functions of the Ophthalmic Technician II (COA) or III (COT) job description relative to certification held by the employee, as defined by HR Policy #2.29--“Technical Training and Competency Expectations.”
- Ability to function as a Clinical Referral Coordinator I as needed
- Maintains and utilizes the referral tracking program within Phreesia
- Contacts patients to schedule their referral appointments
- Coordinates the timely transfer of consult/care related calls to EANM doctors
- Initiates and completes telephone interactions with patients, doctors and others in a professional, courteous and efficient manner
- Responsible for day-to-day review of incoming referrals and medical documentation/records
- Works with department and subspecialty staff and leaders to determine appropriate scheduling timeframes, appropriate subspecialty and further instruction
- May need to document within NextGen the specifics related to the location, symptoms, duration, severity, and other details of ocular complaint
- Communicates with clinic staff when a same-day ER patient is scheduled
- Maintains dialogue with external and internal providers as needed concerning patient’s medications, diagnosis and treatment
- Establishes and sustains accessibility to Tricore lab values for incoming referred patients
- Participates in clinic functions and continuing education opportunities to maintain current clinical certification or pursue higher certification levels according to appropriate certification level guidelines
- Shadowing of EANM physicians as needed to ensure a clear understanding of physician needs and practice preferences
- Stays current with the changes to the Electronic Health Record workflow and updates to the system
- Contributes to continuing performance and process improvement initiatives surrounding referrals, co- management and overall patient access
- Assists with review of incoming records from co-managing providers, communicating with co- managing and EANM providers when patient-specific questions arise, and communicating with Surgery Scheduling staff as needed
- Provides an immediate point of contact for external providers requesting expedited patient care, including facilitation of communication with the EANM on-call physician during normal business hours.
- Participates in both internal and external committees, meetings and / or events as requested by Supervisor of Referral Services and/or Director of Patient Access
- The annual completion of assigned Learning Management System (LMS) courses is mandatory. Any individual who has not completed their assigned training prior to November 30th of each year may be subject to disciplinary action, up to and including termination of employment
Skills/Qualifications
- Position requires high school diploma (or equivalent)
- Minimum certification and clinical experience requirements:
- Certified Ophthalmic Assistant (COA) level with minimum 7 years clinical experience OR Certified Ophthalmic Technician (COT)/Certified Ophthalmic Medical Technician (COMT) level with minimum 3-5 years clinical experience.
- Knowledge of all specialties within EANM encouraged
- Strong verbal and written communication skills, excellent customer service orientation and excellent organizational skills required
- Strong computer skills
- Ability to work as part of a team and highly self-motivated when working independently
- Ability to work effectively with physicians, both internally and externally
- Critical thinking, strong problem solving and clinical decision making required
- High level of professionalism
- Before/after hours meetings and events may be required
- Must be willing to travel to EANM locations as needed
