Job summary

Autumn Hospice is seeking a detail-oriented Clinical Records Coordinator to manage patient medical records from admission through discharge, ensuring accuracy and HIPAA compliance. The role involves coordinating with physicians, facilities, and the interdisciplinary team to maintain complete documentation.

The ideal candidate has experience with electronic and paper records, strong organizational skills, and the ability to work independently while collaborating with an interdisciplinary team.

Qualifications

  • High School Diploma required.
  • Associate Degree preferred.
  • Experience with medical terminology.
  • Experience with electronic and paper medical records.
  • Minimum of one year of related healthcare or medical records experience.
  • Strong organizational, communication, and time management skills.
  • Ability to work independently while collaborating with an interdisciplinary team.

Responsibilities

  • Initiate and maintain patient clinical records from admission through discharge.
  • Prepare, organize, file all required medical record documentation.
  • Ensure patient charts are complete, accurate, and maintained in accordance with policies and regulations.
  • Maintain both electronic and paper medical records while ensuring confidentiality and HIPAA compliance.
  • Track physician orders, certifications, recertifications, plans of care, and other required clinical documents.
  • Coordinate Durable Medical Equipment (DME) orders and communicate equipment issues with clinical leadership.
  • Serve as the primary point of contact for medical record activities between the office, physicians, facilities, and the interdisciplinary team.
  • Follow up with physicians to obtain signed certifications and other required documentation in a timely manner.
  • Retrieve, release, and copy medical records for authorized personnel in accordance with company policies and applicable regulations.
  • Prepare clinical documentation and reports for Interdisciplinary Team (IDT) meetings, including census reports, admissions, discharges, transfers, medication profiles, certifications, and recertifications.
  • Monitor and report patient census information.
  • Participate in daily clinical communication to ensure patient status changes are accurately documented and communicated.
  • Perform routine clinical record audits and promptly identify and resolve missing or incomplete documentation.
  • Support staff in maintaining timely and accurate clinical documentation.
