RD Nutrition Consultants LLC seeks a Clinical Registered Dietitian for a part-time role with a flexible mix of in-person and remote work. This position supports patients in an acute care setting with 12-15 hours per week and only 1 onsite visit required per week.

Qualifications

• Bachelor's or Master's degree in Dietetics, Nutrition, or a related field from an accredited program.
• Registered by the Commission on Dietetic Registration (RD/RDN).
• Current state licensure/certification (if mandated by the state).
• Minimum of 2-3 years of recent clinical experience in an acute care hospital setting (required).

Responsabilités

• Perform comprehensive nutrition assessments for patients identified at nutritional risk, adhering to facility policies and procedures.
• Develop, implement and monitor individualized and evidence-based nutrition care plans for patients with complex medical conditions.
• Offer nutrition education and counseling to patients and families regarding acute disease management, discharge planning, and prevention of readmission.
• Collaborate with physicians, nurses, pharmacists, and other team members to optimize patient outcomes and timely discharge.
• Ensure compliance with hospital policies, regulatory standards, and guidelines for acute care nutrition.
• Maintain thorough and timely documentation in the EHR reflecting medical necessity and intervention effectiveness.
• Monitor patients for weight, labs, fluid status, and clinical changes, adjusting interventions as needed.
