Job summary

Nutrire Nutrition Services is seeking a Registered Dietitian (RD/RDN) or Licensed Dietitian (LD) to provide nutrition services in skilled nursing and long-term care facilities.

Responsibilities include nutrition assessments, monitoring residents at nutritional risk, developing care plans, reviewing weights and laboratory results, completing MDS documentation, and collaborating with the interdisciplinary care team.

Qualifications

• Registration or Florida license required.
• Experience in long-term care preferred.
• Excellent documentation and communication skills.

Responsibilities

• Perform comprehensive nutrition assessments for residents.
• Monitor residents at nutritional risk and adjust care plans accordingly.
• Develop and update individualized care plans.
• Review weight trends and laboratory results to guide interventions.
• Complete MDS documentation accurately and timely.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
