Job summary

Grace Health in Corbin, KY seeks a Clinical Quality Coordinator to lead the quality team and support the Director in coordinating improvement initiatives across the organization. The role emphasizes care coordination, population health, and performance reporting within a PCMH framework.

The coordinator will partner with leadership to track metrics, ensure regulatory compliance, and drive value-based care improvements across Grace Health’s network.

Qualifications

• Degree in Nursing; a Bachelor’s degree preferred.
• Quality Management/Performance improvement experience preferred.
• Community Health Center experience preferred; PCMH experience preferred.

Responsibilities

• Oversee day-to-day activities of the quality department and manage the quality team.
• Coordinate quality improvement initiatives, population health activities, and care gap closure efforts.
• Support quality performance reporting, patient outreach, care coordination, and value-based care performance.
• Collaborate on payer-related quality measures including HEDIS, ACO, CIN, and HRSA OSV activities.
• Maintain communications with department leaders and ensure understanding of PCMH requirements.
