The Program Manager will lead a team of Care Extenders in Mindoula's Interpersonal Violence Reduction Program. They will serve members and deliver outcomes to our health plan partners. The team of Care Extenders empowers members to actively participate in improving their health and wellness through personalized case management tailored to each member’s needs. The Program Manager seeks creative solutions to support Care Extenders and achieve program goals, leading the team toward operational, clinical, and financial outcomes. As a community-based team leader, they model a hands-on approach, empowering the team to meet members where they are and utilize local resources to deliver comprehensive, whole-person care.

100% Remote in the United States.
Monday-Friday 8am to 6pm Central or Eastern time zone.
What you'll do...
- Act as a player/coach, working alongside Care Extenders to build their skills and support members in challenging situations, including:
- Create service plans using all available information about the member, including screenings, conversations, data, and community resources. Guide members in developing skills and strategies for managing problems and triggers to increase relapse prevention and reduce risk.
- Encourage members to develop work skills and participate in social, recreational, or therapeutic activities that enhance interpersonal skills and social relationships.
- Educate members about mental or physical illnesses, abuse, violence prevention, medication, and available community and social resources.
- Monitor, evaluate, and document members’ progress according to measurable goals in their plans.
- Build accountability communications about upcoming appointments.
- Engage continuously with members, families, physicians, and other providers using clinical knowledge to optimize resource utilization.
- Request consultation and diagnostic reports from network specialists as needed.
- Report critical incidents and quality-of-care issues.
- Develop communication strategies to foster engagement and relationship-building with the member cohort to support goal achievement.
- Assess members for risk, perform quick evaluations, and respond within safety plan parameters, including crisis interventions to ensure safety.
- Coordinate the development of community resource guides focusing on medical, behavioral health, violence prevention, and social services.
- Identify behavioral health diagnoses and ensure service plans are comprehensive and appropriate.
- Lead the Care Extender team to ensure member needs are adequately assessed, and all members have appropriate care plans and are progressing toward their goals.
- Coordinate caseloads and assist in referring new members to Care Extenders.
- Ensure team schedules meet program needs.
- Participate in screening, interviewing, and hiring new team members; provide orientation and training; conduct performance reviews; and coach the team.
- Lead enrollment activities to build or maintain the program's cohort.
- Ensure team meets Key Performance Indicators, providing coaching and training as needed.
- Review documentation regularly for quality and training purposes.
- Analyze team performance, identify gaps, and create improvement plans.
- Synthesize data on member outcomes to support training, engagement, and partner relationships.
- Lead internal case review sessions and participate in external meetings with health plan partners.
- Maintain program-related data, reports, and records.
- Partner with billing to review claims and ensure accuracy.
What you'll need...
- A Qualified Mental Health Professional (QMHP) with one of the following licenses, preferably in Nevada:
- Licensed Clinical Social Worker (LCSW)
- Clinical Professional Counselor (CPC) or Licensed Professional Counselor (LPC)
- Licensed Marriage and Family Therapist (LMFT)
- Licensed Psychologist (PhD or PsyD), or equivalent
- If not licensed in Nevada, must be licensed in one of the active program states: Indiana, Louisiana, Tennessee, Texas, or Florida.
- Experience leading, building, and implementing programs with outcomes.
- Strong leadership experience.
- Experience with Health Coaching or Medical Social Work is preferred.
- Master's degree in behavioral health, social work, or healthcare management from an accredited university.
