Job summary

Mentor Community Services, a part of the Sevita family, coordinates the development and implementation of services for individuals with intellectual and developmental disabilities. The role oversees quality of care, compliance with contracts, and supports individuals and families toward greater independence.

The Program Clinical Coordinator works with treatment teams to develop service plans, schedule medical/dental appointments, maintain client records, and assist with staff and mentor training.

Qualifications

• Bachelor’s degree in human services or related field.
• One year of related work experience preferred.
• Equivalent combination of education and experience may be substituted.
• Ohio requires bachelor’s with 1 year experience.
• Pennsylvania IDD: two years experience.
• South Carolina: bachelor’s with three years experience.

Responsibilities

• Coordinate development and implementation of services per standards.
• Oversee service plan development, monitoring, and evaluation.
• Schedule medical and dental appointments for clients.
• Develop and revise programs and behavior plans as needed.
• Provide ongoing client reporting and regulatory compliance oversight.
• Support training for staff and mentors; assist with contracts.

Education

Bachelor’s degree in human services or related field

One year of related work experience preferred

Equivalent combination of education and experience

Ohio: Bachelor's degree + 1 year experience

Pennsylvania: IDD services - 2 years experience

South Carolina: Bachelor's + 3 years experience
