Qualifications

• Licensed physical therapist in MN (or eligible)
• Current BLS or willingness to obtain within 1 month
• Ability to manage a full caseload and coordinate with team

Responsibilities

• Evaluate patients and plan individualized treatment plans.
• Deliver evidence-based interventions and monitor progress.
• Provide patient and caregiver education and discharge planning.
• Handle documentation, scheduling, and other administrative duties for a full caseload
• Collaborate with multidisciplinary care team and coordinate with referring physicians
• Adhere to Essentia Health policies and code of ethics
• Flex to work across departments as needed and participate in committees
