Job summary

Essentia Health in Moorhead, MN is seeking a licensed Physical Therapist to evaluate, plan, treat and implement patient care in line with APTA standards and Essentia Health policies.

Responsibilities include developing individualized treatment plans, delivering evidence‑based interventions, educating patients and caregivers, coordinating with a multidisciplinary team, and managing documentation and scheduling for a full caseload. Clinic hours are 8:00am–5:00pm, Monday through Friday.

Qualifications

• Graduate from an accredited physical therapy program (Bachelor/Master/Doctorate).
• Current licensure to practice physical therapy in the state.

Responsibilities

• Provide therapy evaluation, develop treatment plans, implement interventions and discharge planning for a full patient caseload.
• Handle ancillary duties including documentation, billing, scheduling, and other admin tasks.
• Demonstrate professional communication and supervise support staff as needed.
• Collaborate with a multidisciplinary care team to optimize clinical and financial outcomes.
• Coordinate with Essentia Health leadership and referring physicians for cohesive patient care.

Evaluates, plans, treats and implements care for patients in accordance with professional standards of the American Physical Therapy Association and policies and procedures of Essentia Health.

Education Qualifications:

• Bachelor's Degree, Master's Degree, or Doctorate Degrees from a physical therapy accredited program

Key Responsibilities:

• Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, provide patient/caregiver education, and discharge planning for a full patient caseload.


• Provide all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties


• Demonstrates appropriate communication, professionalism and supervision of support staff (licensed assistants, aides, volunteers)


• Collaborate with a multidisciplinary care team to ensure optimal clinical and financial outcomes.


• Work closely with Essentia Health leadership and coordinate with referring physicians to ensure cohesive patient care.


• Comply with organizational policies and procedures and the code of conduct


• Meet professional organization core values, code of ethics, &/or scope of practice


• Work where the patient need is highest (including flexing to other departments) when home department schedule allows


• May serve as a clinical instructor, participate in department or therapy discipline committees, and complete credential/certification that would benefit patient care

Clinic hours are 8:00am - 5:00pm Monday - Friday. No weekend or holiday rotation required.

Licensure/Certification Qualifications:
