Job summary

Essentia Health's St Josephs - Crosslake Clinic is seeking a Physical Therapist to evaluate, plan, treat, and implement patient care in accordance with professional standards and clinic policies. This role is based in Saint Paul, MN, with a 36-hour work week and occasional weekend rotation.

The candidate must hold a valid PT license in Minnesota, have a degree from an accredited PT program, and maintain CPR/BLS certification.

Qualifications

• Licensed PT in the state performing services.
• Graduated from an accredited physical therapy program.
• BLS certification or ability to obtain within 1 month.

Responsibilities

• Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, educate patients and discharge planning.
• Handle documentation, billing, scheduling management and other administrative duties for a full caseload.
• Communicate professionally and supervise licensed assistants, aides, and volunteers.
• Collaborate with a multidisciplinary care team to optimize clinical and financial outcomes.
• Coordinate with Essentia Health leadership and referring physicians to ensure cohesive care.
