Job summary

Qualifications

• Completion of an accredited PT program with a Bachelors, Masters, or Doctorate in Physical Therapy.
• California Physical Therapist License required at hire or license eligible.
• Basic Life Support (BLS) required at hire.
• National Provider Identifier (NPI) required at hire.

Responsibilities

• Evaluate patients and interpret findings to determine dysfunction and restore functional abilities.
• Establishes plan of care to achieve goals based on age and medical history.
• Administer modalities, exercise programs, and patient care plans.
• Document accurately per state/federal requirements and policies.
• Coordinate discharge planning and participate in rounds and case management.

Education
