Job summary

Northwell Health is seeking a licensed Physical Therapist to evaluate and treat patients under physician prescriptions and referrals. You will maintain medical records and design treatment plans to help patients achieve established goals.

Responsibilities include monitoring patient progress, ensuring safety, and collaborating with the inter-disciplinary team to provide high-quality, client-centered care in a hospital or clinical setting.

Qualifications

• Master's degree in Physical Therapy is required.
• Current license to practice as a Physical Therapist in New York State and/or Connecticut is required.
• 1–3 years of relevant experience preferred.

Responsibilities

• Evaluates and treats patients in accordance with prescriptions and referrals.
• Maintains complete medical records for each patient.
• Designs individualized treatment programs to achieve goals.
• Monitors patient tolerance/status and modifies plans as needed.
• Responds to department emergency procedures to ensure safety.
• Submits required documentation per department policies and procedures.
• Participates in quality improvement activities and chart reviews.
• Attends family meetings and collaborates with inter-disciplinary staff.
