Job summary

Confluent Health is seeking a licensed Physical Therapist for an on-site role in Illinois (Mattoon). Compensation ranges from $72,000 to $90,000 per year, with a sign-on bonus up to $10,000 and comprehensive benefits including PTO, medical, dental, vision, disability, and a 401(k) with a 4% employer match.

Qualifications

• Licensed Physical Therapist (or ability to obtain licensure in the state of practice).

Responsibilities

• Assess movement dysfunction and determine underlying causes from history and exam.
• Collaboratively develop and adjust evidence-based plans of care to meet patient goals.
• Deliver skilled interventions including exercise, manual therapy, neuromuscular re-education, and modalities.
• Track patient outcomes and adjust approach based on progress.
• Act as a trusted resource and advocate for patients throughout recovery.
