Job summary

Sentara Health in Stafford, VA is seeking a dedicated Physical Therapist to assess, plan, and participate in rehabilitative programs that improve mobility and reduce pain. This role involves educating clients and caregivers and supervising care by the assigned team members.

The candidate will collaborate with an interdisciplinary team, develop and maintain a formal care plan, and ensure interventions align with department standards.

Qualifications

• Performs modalities/therapeutic interventions in accordance with accepted standards of care and department policies and procedures.
• Provides effective instruction to the patient and family through standardized educational programs and/or individualized training.
• Initiate, develop, and maintain collaborative interactions with internal and external team members, patients, and family members to improve patient outcomes. Productive and efficient in daily operations.
• Selects and performs the appropriate assessment procedures. Establishes and updates formal plan of care to address individual patient needs.

• Assess, plan, organize, and participate in rehabilitative programs that improve functional mobility and relieve pain.
