Job summary

Chautauqua Health & Rehabilitation is seeking a licensed Physical Therapist to provide therapy services, screen and evaluate patients, and develop treatment plans within organizational guidelines. • Supervise PTAs, Rehab Techs, and/or Physical Therapy students in accordance with state practice act guidelines, providing consultation and clinical supervision and supporting clinical development in collaboration with the Director of Rehabilitation.

Responsibilities

• Provide physical therapy services and patient evaluations, develop treatment plans.
• Supervise PTAs and Rehab Techs; coordinate with rehab team.
• Ensure accurate documentation and billing per regulations.
• Participate in care conferences and interdisciplinary team activities.
• Maintain licensure and uphold professional standards.
