Job summary

Luminis Health, Inc. in Annapolis, MD is seeking a Clinical Pharmacist to provide evidence-based pharmacotherapy and patient-centered pharmaceutical care within the hospital pharmacy.

You will develop medication management protocols and participate in drug therapy policies. The role emphasizes collaboration with multidisciplinary teams, clinical rounds, medication reconciliation, and education of staff and students.

Qualifications

• The minimum education is the Doctor of Pharmacy degree.
• Residency training or demonstrated clinical experience is required.
• Advanced residency or board certification is preferred.

Responsibilities

• Conduct department activities to meet pharmacy goals and policies.
• Improve interdepartmental policies and procedures aligning with hospital strategy.
• Complete core competencies and document clinical activities per policy.
• Act as medication expert on medical/surgical units for time-sensitive guidance.
• Provide pharmacotherapy guidance to optimize patient care and reduce readmissions.
• Perform drug class reviews, formulary changes, and safety activities.
• Assist with medication reconciliation and discharge education for high-risk patients.
• Educate staff and residents on approved guidelines and cost-effective use of meds.

Job description

Position Objective:

Responsible for carrying out Clinical Pharmacy services through promoting evidence-based pharmacotherapy and sound pharmaceutical care through the development of medication management protocols, and drug therapy policies. Serves as a Clinical Pharmacy resource for the health system.

Essential Job Duties:

1. Conducts departmental activities to meet the goals, objectives and policies of the pharmacy department.


2. Participates in improvements in interdepartmental policies and procedures, which support the Pharmacy Department and Hospitals strategic goals. Maintains accurate records as required by departmental policies and procedures and/or federal and state board regulations.


3. Completes all core competencies and documents clinical activities accurately and according to policies and procedures. Supports the Investigational Drug Service in the daily operations of the department.


4. Serves as the medication expert on medical and surgical units and is available to provide time-sensitive, accurate information for other healthcare providers in acute scenarios.


5. Provides pharmaceutical care, dosing and guideline recommendations to impact patient care and decrease readmissions. Provides guidance for drug selection and pharmacotherapy regimens in accordance with evidence-based guidelines.


6. Completes pharmacotherapy drug class reviews, formulary addition/deletion, assimilation of evidence-based protocol/order-sets, medication safety related activities and/or targeted medication expense reductions.


7. Assists in appropriate transition of care services, including medication reconciliation and discharge education of high-risk patients (or those with pertinent medication-related problems) as appropriate.


8. Serves as the pharmacy representative on multidisciplinary committees and develops pharmacotherapy-specific specific educational programs, protocols, standard operating procedures, and order sets.


9. Educates other healthcare providers, via formal and informal in-services, on approved treatment guidelines and criteria to increase appropriate and cost-effective use of medications in the medical and surgical units.


10. Serves as a resource for Pharmacy School Students and AAMC Pharmacy Residents.

Educational/Experience Requirements:

The minimum level of education and experience for this position includes:

• Doctor of Pharmacy degree required


• Completion of PGY-1 residency or 2 years of demonstrated clinical experience required


• Completion of PGY-2 residency or board certification in pharmacotherapy preferred

Required License/Certifications:

• Current Maryland State License

Working Conditions, Equipment, Physical Demands:

• Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.

Pay Range

$62.13—$93.19 USD
