Job summary

Luminis Health is seeking a Clinical Staff Pharmacist to support safe, high-quality drug use and cost-effective care in an inpatient setting. The role involves dispensing, verifying orders, and collaborating with clinicians to optimize therapy.

The position requires a Doctor of Pharmacy degree or a BS in Pharmacy with relevant experience, and Maryland licensure. PGY-1 or board certification is preferred, with a rotating schedule and inpatient responsibilities.

Responsibilities

  • Verify accuracy of order entry by prescribers in patient profiles.
  • Review medication orders for ingredients, strength, dosage, interactions, and adverse effects.
  • Supervise Pharmacy Technicians to ensure accurate and timely dispensing of medications.
  • Participate in patient care rounds and unit-based services to improve safety and cost efficiency.
  • Educate staff and patients on drug information and therapies as appropriate.

Job description

Position Objective:

Under general supervision, the Clinical Staff Pharmacist facilitates the safe, high quality, effective and economic use of drugs. This responsibility includes the accurate preparation and dispensing of pharmaceuticals according to established departmental policies and procedures.

Essential Job Duties:
  • Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
  • Partakes in a rotating staffing schedule, including day and evening shifts, weekend rotations, holidays, and overnight coverage based on departmental needs.
  • Verifies the accuracy of order entry by authorized prescribers in computerized patient medication profiles.
  • Reviews written medication orders or prescriptions to determine appropriate ingredients, strength, and dosage. Reviews medication orders for: potential allergies, drug/drug and food/drug interactions, route of administration, appropriate dose and potential for adverse drug reactions with other medications ordered.
  • Contacts the prescriber, as appropriate, to clarify orders and/or make recommendations concerning alternate therapies.
  • Provides drug information to patients, physicians, nurses, and other clinical staff where appropriate and upon request.
  • Supervises the daily operational duties in the dispensing area to which assigned, including assisting and assigning, closely monitoring and reviewing the work of Pharmacy Technicians, to assure accurate and timely delivery of medications.
  • Participates in patient care rounds and unit based services, which may include: medication reconciliation, promoting medication efficacy/safety/cost improvement initiatives, participation in unit process improvement activities, and patient education & follow-up as appropriate.
  • Participates in improvements in interdepartmental policies and procedures which support the Pharmacy Department and Hospitals strategic goals. Maintains accurate records as required by departmental policies and procedures and/or federal and state board regulations.
  • Completes all core competencies and documents clinical activities accurately and according to policies and procedures. Supports the Investigational Drug Service in the daily operations of the department.
  • Educates and supervises Pharmacy Technician in the preparation of intravenous admixture solutions and chemotherapeutic agents, using aseptic technique.
  • Completes formal in-service, formulary addition/deletion, assimilation of evidence-based protocol/order-set, medication safety related activities and/or targeted medication expense reductions.
Educational/Experience Requirements:

The minimum level of education and experience for this position includes:

  • Doctor of Pharmacy Degree or Bachelor of Science in Pharmacy with commensurate experience required.
  • At least 1 year of experience in an inpatient setting required.
  • PGY-1 residency or Board Certification preferred.
Required License/Certifications:
  • Current Maryland State License
Working Conditions, Equipment, Physical Demands:

Light work. Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly to move objects. If the use of arm and/or leg controls requires exertion of forces greater than that for sedentary work and the worker sits most of the time, the job is rated for light work.

There is a reasonable expectation that employees in this position will be exposed to blood-borne pathogens.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
