Job summary

Luminis Health in Annapolis, MD, seeks a clinical pharmacist to deliver Pharmacy services, promote evidence-based pharmacotherapy, and develop medication management protocols and drug therapy policies across the health system.

You will serve as a resource on medical and surgical units, provide dosing guidance, participate in formulary management, transitions of care, and educate staff. Requires a Doctor of Pharmacy degree, PGY-1 residency, and Maryland license.

Qualifications

  • Doctor of Pharmacy degree required.
  • Completion of PGY-1 residency or 2 years of demonstrated clinical experience required.
  • Completion of PGY-2 residency or board certification in pharmacotherapy preferred.

Responsibilities

  • Serves as the medication expert on medical and surgical units and is available to provide time-sensitive, accurate information for other healthcare providers in acute scenarios.
  • Provides pharmaceutical care, dosing and guideline recommendations to impact patient care and decrease readmissions.
  • Provides guidance for drug selection and pharmacotherapy regimens in accordance with evidence-based guidelines.
  • Assists in appropriate transition of care services, including medication reconciliation and discharge education of high-risk patients.
  • Serves as the pharmacy representative on multidisciplinary committees and develops pharmacotherapy-specific educational programs, protocols, SOPs, and order sets.
  • Educates other healthcare providers on approved treatment guidelines to increase appropriate and cost-effective use of medications.
  • Serves as a resource for Pharmacy School Students and AAMC Pharmacy Residents.

Education

Doctor of Pharmacy degree
PGY-1 residency
PGY-2 residency or pharmacotherapy board certification preferred

Job description

Position Objective

Responsible for carrying out Clinical Pharmacy services through promoting evidence-based pharmacotherapy and sound pharmaceutical care through the development of medication management protocols, and drug therapy policies. Serves as a Clinical Pharmacy resource for the health system.

  • Conducts departmental activities to meet the goals, objectives and policies of the pharmacy department.
  • Participates in improvements in interdepartmental policies and procedures, which support the Pharmacy Department and Hospitals strategic goals. Maintains accurate records as required by departmental policies and procedures and/or federal and state board regulations.
  • Completes all core competencies and documents clinical activities accurately and according to policies and procedures. Supports the Investigational Drug Service in the daily operations of the department.
  • Serves as the medication expert on medical and surgical units and is available to provide time-sensitive, accurate information for other healthcare providers in acute scenarios.
  • Provides pharmaceutical care, dosing and guideline recommendations to impact patient care and decrease readmissions. Provides guidance for drug selection and pharmacotherapy regimens in accordance with evidence-based guidelines.
  • Completes pharmacotherapy drug class reviews, formulary addition/deletion, assimilation of evidence-based protocol/order-sets, medication safety related activities and/or targeted medication expense reductions.
  • Assists in appropriate transition of care services, including medication reconciliation and discharge education of high-risk patients (or those with pertinent medication-related problems) as appropriate.
  • Serves as the pharmacy representative on multidisciplinary committees and develops pharmacotherapy-specific specific educational programs, protocols, standard operating procedures, and order sets.
  • Educates other healthcare providers, via formal and informal in-services, on approved treatment guidelines and criteria to increase appropriate and cost-effective use of medications in the medical and surgical units.
  • Serves as a resource for Pharmacy School Students and AAMC Pharmacy Residents.
Educational/Experience Requirements
  • Doctor of Pharmacy degree required
  • Completion of PGY-1 residency or 2 years of demonstrated clinical experience required
  • Completion of PGY-2 residency or board certification in pharmacotherapy preferred
Required License/Certifications
  • Current Maryland State License

Pay Range

$62.13—$93.19 USD
