HOURLY RANGE: $61.81- $86.53

DISTINGUISHING FEATURES OF THE CLASS: The work involves providing patient focused pharmacy services that promote the safe, effective, judicious and cost-effective use of pharmaceuticals at the Erie County Medical Center Corporation. The incumbent collaborates with the patient care team to improve the medication use process with the goal of achieving significant advances in the provision of safe, cost effective and appropriate use of medications to promote optimal patient outcomes. Work is performed under the general supervision of a higher-level licensed Pharmacist. Supervision is exercised over staff pharmacists, interns, residents and students. Does related work as required.

TYPICAL WORK ACTIVITIES
  • Assists in planning, organizing, implementing and evaluating clinical pharmacy programs;
  • Utilizes clinical knowledge and guidelines to apply best practice models to patient directed pharmacotherapy;
  • Advises the prescriber, nurse and patient regarding the contents, therapeutic action, utilization and possible adverse reactions or interactions of prescribed drugs;
  • Documents cost savings/avoidance and patient care interventions;
  • Works with applicable departmental staff to track and manage pharmaceutical utilization, costs and prescribing trends to identify opportunities for improvement and outcome measures of related initiatives;
  • Develops and implements pharmacy staff development programs to support clinical service initiatives and promote the professional growth of the pharmacists;
  • Develops educational programs for medical and nursing staff to promote optimal medication utilization, positive patient outcomes and cost containment initiatives;
  • Participates in patient care rounds to facilitate a proactive approach to optimal pharmaceutical management;
  • Participates in clinical research and primary literature publications;
  • Participates in the orientation of new staff; serves as preceptor/mentor of pharmacy students, interns and residents;
  • Participates in staff performance evaluations; develops clinical competencies;
  • Delegates assignments to staff that are within their scope of practice and provides the maximum utilization of personnel; researches new concepts and patterns for innovative staffing patterns;
  • Completes various reports for department and hospital use;
  • Serves on various committees.

FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of modern principles, procedures, terminology and concepts of pharmacology; thorough knowledge of pharmacy procedures and techniques in hospital, long-term care and community pharmacy settings; thorough knowledge of applicable laws, rules, regulations and accreditation standards associated with pharmaceutical drugs and pharmacy operations; ability to perform a variety of professional pharmaceutical tasks with a high degree of accuracy; ability to implement programs and procedures; ability to maintain detailed pharmaceutical records and prepare reports; ability to supervise and train others; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with a diverse constituency; ability to utilize a variety of electronic software applications; sound professional judgment; capable of performing the essential functions of the position with or without reasonable accommodation.

MINIMUM QUALIFICATIONS

Graduation from a regionally accredited or New York State registered school of pharmacy; and both:

  • One (1) year of clinical pharmacy experience*; and:
  • Completion of one (1) year of an approved post-graduate pharmacy residency as accredited by the American Society of Health-System Pharmacists (ASHP).

*NOTE 1: One (1) year clinical pharmacy experience may be waived if the applicant has either:

  • Certification in clinical pharmacy**; or:
  • Completion of two (2) years of an approved post-graduate pharmacy residency as accredited by the American Society of Health-System Pharmacists (ASHP).

**NOTE 2: Certification must be maintained throughout duration of appointment.

SPECIAL REQUIREMENTS

Possession of a license to practice as a Pharmacist as issued by the New York State Department of Education at time of appointment and maintenance of licensure throughout duration of appointment; or: eligibility for licensure by reciprocation as a Pharmacist with the New York State Department of Education and full licensure within three (3) months of appointment and maintenance of licensure throughout duration of appointment; and:

Certification in clinical pharmacy within six (6) months of appointment and maintenance throughout duration of appointment.

NOTE 3: Verifiable part-time and/or volunteer experience will be pro-rated toward meeting full-time experience requirements.
